Application Guidelines

Interested students must submit an application between December 1 and February 1 for funding in the upcoming fall semester. Students must complete the application available at the bottom of this Web page and mail it along with a current, complete official transcript of grades to Scholarship America postmarked no later than February 1.

Online transcripts and grade reports are not acceptable.

Applicants are responsible for gathering and submitting all necessary information. Applications are evaluated on the information supplied; therefore, it is important to answer all questions as completely as possible. All information received is considered confidential and is reviewed only by Scholarship America.

Upon receipt of your application, Scholarship America will mail a confirmation card to your home address. This card will give the deadlines and detailed instructions for completing the application process. If you do not receive a confirmation card within three weeks from the date you mailed your original application, please call Scholarship America and ask for the Humana Foundation Scholarship Program Manager.

You can view and print the scholarship brochure and application as a document suitable for printing or a PDF fill-in form. The application is updated on an annual basis, so be sure to print the latest application below before submitting.
